Las Vegas Metropolitan Police Department officers responded to a welfare check at the Rio Hotel and Casino around 10:45 a.m. on Sunday.

Police knocked repeatedly and called out to the mother and daughter, but there was no response, Lt. Robert Price said during a press conference.

After officers cleared the scene, hotel security continued receiving urgent calls from concerned family and friends. Eventually, security entered the room.

Inside, they discovered a woman in her mid-30s and a preteen girl deceased.

UXC Coaches identified the deceased as Tawnia McGeehan and her 11-year-old daughter, Addi Smith. Police believe the deaths were an apparent murder-suicide, according to ABC4.

“This is a sad and tragic incident, and our hearts go out to the family,” police reportedly said during the press conference. The investigation remains ongoing.

Why the Loss Is Hitting So Hard

Addi wasn’t just another athlete. In an emotional interview cited by ABC4, Utah Xtreme Cheer gym owner Kory Uyetake described her as the team’s spark.

Addi “was kind of a hype man on the team, she was always the person to get up there and get the kids pumped up when things weren’t really going their way.”

“She was an athlete that just embodied everything that we that we [sic] strive to have and we strive to do and teach our kids at my gym.”

The cheer organization later shared a heartbreaking tribute on Instagram, writing, “With the heaviest hearts, we share the devastating news that our sweet athlete Addi has passed away.”

“We are completely heartbroken. No words do the situation justice. She was so beyond loved, and she will always be a part of the UXC family,” the post read on.

The Custody Battle Behind the Scenes

As investigators work to piece together what happened inside the hotel room, court documents have revealed a nearly decade-long custody dispute between Tawnia and Addi’s father, Bradley Smith.

According to court records reported by TMZ, the former couple’s legal battle began after Tawnia filed for divorce in 2015. Temporary orders initially granted Tawnia primary residence for Addi, with Bradley receiving parent time and joint custody.

In 2020, however, a temporary order accused Tawnia of subjecting Addi to “behavior on the spectrum of parental alienation.” The documents also alleged that domestic abuse occurred in front of the child.

That same year, Bradley was awarded temporary sole custody after reportedly demonstrating “a better understanding of the child’s needs.” In 2021, Tawnia filed a temporary restraining order, alleging Addi’s stepmother was recording her during custody exchanges.

The custody exchanges themselves were tightly regulated, according to court documents cited by ABC4. Initially conducted at Addi’s school, they were later required to take place at a police station when school was not in session. The divorce decree was finalized and signed by a judge in 2024.

In the end, Tawnia and Bradley were awarded joint legal and physical custody on a one-week-on, one-week-off schedule in 2024, according to TMZ. Multiple accusations of custodial interference against Tawnia were later dismissed, TMZ reported.

The Tragic Discovery

Spencer Tepe, 37, a respected dentist at Athens Dental Depot, failed to show up for work that Tuesday. His absence immediately alarmed colleagues and his employer, Dr. Mark Valrose, who said Spencer was always on time and would have called if something were wrong.

At approximately 9:03 a.m., Dr. Valrose reportedly called 911, telling dispatchers that no one could reach Spencer or his wife, Monique, 39.

The friend’s 911 call at 9:57 a.m. was soon followed by another, as just minutes later, he had made a horrifying discovery: Spencer’s body was near a bed. The friend told the 911 operator that there was a body in the home and that Spencer appeared to be dead.

When other officers arrived, they entered the property and confirmed that both Spencer and Monique were dead. Their two young children had been left unharmed.

What Happened to the Tepes?

Shreveport police officers shot and killed him on Brompton Lane near Long Acre Drive. He was carrying a small-caliber handgun and, when confronted by officers, had a rifle-style pistol. In total, ten people were shot that morning — eight of them fatally.

A City Lost for Words

This was the deadliest mass shooting in the United States since eight people were killed in a Chicago suburb in January 2024. Shreveport, a city of roughly 180,000 people, had never seen anything like it.

Shreveport Police Chief Wayne Smith stood before cameras, visibly shaken. “I just don’t know what to say, my heart is just taken aback,” he said. “I cannot begin to imagine how such an event could occur.” Mayor Tom Arceneaux called it “the worst tragic situation we’ve ever had.”

That evening, mourners gathered outside the house on West 79th Street and laid flowers. One door appeared stained with blood. At a nearby prayer vigil, community members lit candles in a parking lot.

Kimberlin Jackson, who was there, put it plainly: “It just makes you take your children and hug them and hold them and tell them how much you love them.”

What the Neighbor Heard – and Didn’t

Liza Demming lives two doors down from the West 79th Street house. Her security camera captured Elkins running from the home toward a nearby tire shop. The audio caught two shots.

But before that? Nothing. “It was nothing loud, no altercations,” she told reporters. “It was quiet. Nothing.” To anyone on that street, there was no warning at all.

Inside the investigation, though, a very different picture was forming.

The Marriage That Was Already Over

Detectives confirmed the attack was entirely domestic in nature. And once family members began speaking, the shape of what led up to it became clearer.

Elkins and his wife had been in the middle of a separation. They were due in court the very next day — Monday. According to that account, the couple had been arguing about the separation before the shooting.

That detail shifted the timeline.

It placed a domestic argument directly before the violence, narrowing the focus to those final hours inside the home.

The relative said, “He murdered his children. He shot his wife.”

Elkins shared four children with his wife, and three with another woman who lived nearby, and all their children had been together that night at the house on West 79th Street. The eighth child who was shot was described as a relative.

The two women shot that morning were his wife, who was shot in the face and survived, and the second woman, believed to be the mother of three of his children. Both were critically wounded.

A Record That Should Have Raised Flags

Shreveport police acknowledged they were already familiar with Elkins — but said they had no known history of domestic violence in the home. What they did have on file was something else entirely.

In March 2019, Elkins was arrested on two charges: illegal use of weapons and carrying a firearm on school property.

His own account, documented in a police report, was that he had stepped outside to meet a friend when someone in the car pulled a gun on him first — and he fired back, five rounds, as the car drove away.

The location of that shooting was less than 300 feet from the fence line of Caddo Magnet School. The rounds were fired in the direction of the building while children were playing outside.

Elkins pleaded guilty to the weapons charge in October 2019. The second charge was dismissed, and he was sentenced to 18 months of probation.

Domestic Violence Is Not a Private Matter

Caddo Parish District Attorney James Stewart did not let the morning pass without naming what it exposed. “What began as a domestic dispute has ended in irreversible harm,” his office stated.

“Domestic violence is not a private matter. It is a community issue with far-reaching consequences, often affecting the most vulnerable among us — our children.”

And as recent headlines have shown, this pattern — a domestic situation under strain, followed by sudden violence — is not isolated to one city or one case.

In fact, just days earlier, another story involving a well-known public figure revealed a similarly complex timeline, where court filings, financial strain, and a looming separation raised difficult questions about what happens behind closed doors.

At first, it looked like another devastating headline. But as the pieces fall into place, something far more unsettling begins to emerge. Because behind the quiet exterior of a family home, a storm had clearly been building for some time.

Pressure Inside the House

To understand how things reached this point, attention quickly turns to what was happening inside the home in the weeks leading up to the tragedy. The first clues were chaotic, incomplete, and terrifying. But as the details spilled out, the final days inside Justin Fairfax’s home began to look even darker than anyone first realized.

At first, all anyone seemed to know was that something awful had happened behind closed doors. Then came the 911 call, and with it, a series of details that turned a shocking death into a deeply unsettling unraveling.

A report from AP News revealed one detail that now stands out: in a March 30 order, a judge told Justin to move out of the family home by the end of April. That means the killing happened weeks before the deadline he had been given to leave.

That timing is impossible to ignore, especially because the divorce proceedings, by then, appear to have turned into a brutal mix of emotional collapse, financial strain, and life inside a house that neither side had fully escaped.

As the court filings show, the tension was not just emotional — it was deeply practical, too.

AP reported that Cerina told the court her husband drank daily, and that his area of the home was littered with empty wine bottles and dirty laundry. Court records also showed that he bought a handgun in 2022 with money intended for their children’s horseback riding lessons.

The couple’s finances had reportedly become a serious source of conflict, too. As People reported from the divorce filings, Cerina claimed Justin failed to pay his 50% share of their children’s private school tuition and extracurricular activities, while also not covering family groceries, utilities, or the mortgage.

In one especially stark line, she wrote, “Husband does not make any financial contributions to support the Wife and their children.” She said all financial responsibilities were being carried by her alone.

Cerina also accused him of allowing the marital home to go into foreclosure while spending marital funds on extravagant trips for third parties. On top of that, civil court records showed Justin had defaulted on almost $30,000 in credit card debt in 2025.

So by the time the final court fight was playing out, this was not simply a marriage ending; it was, at least according to the filings, a household strained by money problems, unpaid family obligations, debt, and a growing sense that the floor was giving way.

The emotional picture painted in the court battle was just as grim. In reporting on those filings, the New York Post said Cerina alleged that after Justin left office in 2022, he withdrew even further from his family and that his troubling behavior had “not abated to this day.”

A Family Rift That Kept Deepening

The filing described him as “a talented man who struggles with undefined emotional and psychological issues.” It also said there could “be no mistake” that those issues were defining and limiting him, including in his role as a father.

The same court material described his isolation, drinking, and lack of participation in family life as signs of what seemed to be “a sense of fatalism and hopelessness.”

The Post also noted that Cerina, after 20 years of marriage, filed for divorce in July 2025, a year and a day after the couple initially separated. Yet despite the separation, they were still living together while the divorce dragged on.

That living arrangement, in hindsight, now feels especially significant.

And then there is the episode that suggested things had already reached a frightening point years earlier. According to a Washington Post report citing court documents, Cerina testified that Justin’s “heavy daily alcohol” use became a “new normal” after he left office in 2022, and that he grew more insular, spending more time alone in his room rather than with the family.

That same year, according to the documents, he bought the gun and at one point left home with the firearm and a suitcase packed with clothes. Cerina, her stepfather, and another relative then went looking for him.

When they later found him in a public park near the family’s home, he reportedly said the gun was for personal protection and that he had taken it because he did not want his children to find it at home.

The morning after, Justin’s brother arranged for a mental health professional to speak with him. AP’s reporting adds more context to how far Justin had fallen from his earlier public image.

A Paper Trail Full of Red Flags

Stepping back further, the broader timeline only adds to the sense that pressure had been building for years. Less than a decade ago, he was widely seen as a rising Democratic star, having won the race for Virginia lieutenant governor in 2017 and appearing positioned for even stronger political possibilities.

Then came the sexual assault allegations in 2019, which he denied, insisting the encounters were consensual. He refused calls to resign, later tried to run for governor in 2021, and lost badly after being largely shunned by Virginia Democrats.

That career collapse appears to have echoed through everything else. AP reported that one of Justin’s longtime friends said he became increasingly despondent after Cerina filed for divorce, and a judge wrote that his “mental and emotional health” had suffered after both his failed 2013 attorney general bid and the 2019 scandal, though, after the latter, he never truly bounced back.

AP also reported that friend Sophia A. Nelson said Justin never moved past the scandal. She said that in a group text just two days before the killings, he brought up recent allegations against Rep. Eric Swalwell because it revived, for him, what he still believed had been an unfair rush to judgment in his own case.

Sophia said she and others repeatedly urged him to get help. “There were talks of suicidal thoughts,” she told the AP. That concern, in hindsight, now feels especially chilling.

She also said she had urged him to move out, but believed he may not have been able to do so financially. And the money problems were not small.

AP reported that filings showed Justin struggled financially after the allegations cost him his position as a partner at a prestigious law firm, and that the IRS had previously filed a lien against the couple for over $91,000 in unpaid taxes, though it was resolved in 2021.

Sophia bluntly told AP that Justin had become “unemployable” after the scandal. He reportedly tried to rebuild his legal career, but only with limited success.

That makes the final timeline feel even more claustrophobic. A failing marriage, unpaid obligations, debt, a court battle, worsening isolation, drinking, and a deadline to get out of the house — all of it closing in at once.

And then come the details that reframe everything. For much of this story, the focus has been on the unraveling marriage, the finances, and the personal struggles.

But two details, confirmed by authorities, shift the weight of the tragedy in a profound way. According to AP News, officers found the estranged couple dead inside their Annandale, Virginia, home early Thursday, April 16, 2026.

Reporting from NBC News further revealed that the couple’s teenage children were inside the home at the time of the shootings. Their older son, Cameron Fairfax, was the one who called 911 just after midnight. And as more information emerged, that moment became even more harrowing.

A witness is now speaking out about the desperate final moments of a grandmother who vanished through an open manhole in the middle of New York City. What he heard her cry out from the bottom of that hole is something he will not soon forget.

Donike Gocaj, 56, a grandmother from Briarcliff Manor in Westchester County, had just parked her Mercedes on a Manhattan street on the night of Monday, May 18, 2026, less than a block from one of the most expensive storefronts in the city. She never made it to her destination.

Where Was She, and What Did She Walk Into?

The stretch of East 52nd Street where Gocaj parked sits directly outside Cartier’s flagship New York City store, one of Midtown Manhattan’s most recognizable blocks. It was nearly 11:20 p.m., the street was dark, and there was nothing to warn her of what lay just beyond her car door.

Fire safety director Carlton Wood was on his way to work when he watched Gocaj step out of her vehicle, close the door behind her, and take a single step forward.

A moment later, she dropped 10 feet into an uncovered drain below, disappearing from sight instantly. Of course, Wood’s instincts kicked right in, and he ran toward the hole.

He found her sitting at the bottom in a steaming hot puddle of water, legs stretched forward, looking up and begging for help.

What Did Bystanders Do to Try to Save Her?

Wood called 911 while other witnesses at the scene scrambled to act. Several people attempted to lower a man into the hole by his arms, hoping Gocaj could grab onto his feet and be pulled to safety, but the depth made it impossible.

Someone retrieved a ladder from their car, only to find it was too large to fit into the opening. Every attempt to reach her came up short, and the clock was moving. Through all of it, Wood could hear her voice rising from the darkness below.

By the time he got off the phone with 911 a few minutes later and rejoined the group, trying to hold the would-be rescuer steady, Gocaj had been repeating the same thing over and over: “I’m dying, I’m dying.” Those were the last words anyone would hear her speak.

What Happened When Emergency Services Arrived?

Gocaj had lost consciousness before paramedics and the FDNY even arrived on scene. Firefighters spent roughly 20 minutes extracting her from the manhole.

When she was finally brought out and placed on a stretcher, Wood said her legs appeared black and badly burned, as though they had been severely scalded by the steaming water below.

The grandmother was transported to New York-Presbyterian/Weill Cornell Medical Center, where she was unfortunately pronounced dead. The Office of the Chief Medical Examiner of the City of New York will determine her official cause of death.

Why Was the Manhole Left Uncovered?

The manhole cover was found lying about 15 feet away from the opening. There were no safety warning devices, according to Wood. He also made it clear that Gocaj didn’t do anything wrong.

“There were no cones, no barricades, no signs. She wasn’t on her phone and didn’t seem distracted at all. It was dark and unfortunately she just didn’t see the hole. It could have happened to anyone.”

Gocaj’s family, visibly shaken at the scene the following day, echoed that account. Her daughter-in-law told CBS there were absolutely no barriers or signage around the open hole, and the family demanded answers about how it had been left that way.

New York City baked in 86°F heat that Monday, which sparked speculation about whether the temperature could have caused the cover to dislodge on its own.

But Con Edison, the company that maintains tens of thousands of manholes across the city, later reviewed surveillance footage and landed on a different explanation entirely.

What Did Con Edison’s Investigation Reveal?

According to a statement obtained by People, the cover was dislodged when a multi-axle truck turning from 5th Avenue onto 52nd Street drove over it. Approximately 12 minutes later, Gocaj parked her car nearby and stepped out into the unguarded opening.

“We are deeply saddened to confirm that a member of the public has died after falling into an open manhole. We are actively investigating how this occurred,” Con Edison said, adding that its thoughts remain with her family and that safety is its top priority.

City regulations require that any uncovered street openings be barricaded and clearly marked with flags, lights, or signs. Contractors are also mandated to temporarily cover any manholes being worked on outside of regular hours.

Whether any work had been conducted at that location in the hours before Gocaj fell remains unclear, but no criminal activity is suspected at this time.

The New York City Department of Environmental Protection manages roughly 100,000 active manholes across the city. The New York Times reports that the department has already received more than 700 service requests regarding open manholes so far in 2026.

Workers have since covered the gap with a metal grate, and barriers and cones have been placed around the scene.

What Did City Officials Say?

The Mayor’s Office, currently held by Zohran Mamdani, addressed the tragedy directly, and its statement left little room for ambiguity about the city’s responsibility to follow through.

“Our condolences are with the family of the woman who lost her life in this devastating incident,” the office said. “City agencies are working with Con Ed to support the emergency response and conduct a full investigation into what occurred. Every question must be asked and answered so that no New Yorker experiences a tragedy like this again.”

Unfortunately, a grandmother lost her life earlier this year in another unexpected situation. A Georgia family started demanding answers after a grandmother attending a wake was killed in a drive-by shooting outside the Gus Thornhill Funeral Home in East Point on Friday, January 9.

According to authorities, 55-year-old Ina Chambers was fatally shot during the attack, which also left another woman injured. As of Sunday, January 11, evening, no arrests had been made, and police said multiple suspects were involved in the shooting.

When and Where the Shooting Occurred

The shooting happened outside the East Point funeral home, where several wakes were taking place simultaneously.

East Point Police Chief Shawn Buchanan described the incident as an “outrage,” explaining that four different families were inside the building mourning loved ones when gunfire broke out.

“They’re suffering. They’re trying to bury their loved ones, and for someone to take that opportunity away from them and destroy their peace… our team will not rest until justice is served,” the police chief said.

Who Chambers was

Two days after her death, Chambers’ family gathered with friends outside their DeKalb County home, releasing balloons as they struggled to process the sudden loss.

“They came for her,” her husband, Phil Chambers, said when speaking to WSAW-TV about the shooting. “I just got a clear mind. Don’t know what to do,” he shared. “I lost my wife,” he added when speaking to Fox5 Atlanta.

Her older brother, Efrem Jenkins, remembered her as the center of every gathering. “She was vibrant. She was the life of the party,” Jenkins said.

In an interview with Fox 5 Atlanta, he added that his sister was “vibrant” and “very biblical,” a devoted wife who was always doing something good for others.

“My little sister has been taken away,” he continued. “For those of you who know her, you know she’s the best.”

Why the Loss Has Shaken Loved Ones

Jenkins said the family was still trying to come to terms with the tragedy because “It’s so senseless, and it’s just hard to wrap your head around it,” Jenkins continued. “The phrase that we’ve been using is just: no words. Just no words.”

He also issued a message to those responsible for the violence:

“Just no words. The family is small, and it’s getting even smaller by the day. Gun control is one thing, but control your kids. And if they’re not kids, they’re young adults: listen, you’re ruining your lives, and you’re taking our lives.”

“If you’re out there doing senseless things like this, just know the world goes in a circle and it does come back around, you just never know how. So, please stop,” Jenkins added.

A Celebration That Gave Way to an Emergency

Wednesday, July 29, should have belonged to Stephen and Tracey. According to their Facebook profiles, the couple appeared to have reached 40 years of marriage that day, a milestone one of Tracey’s friends also acknowledged while remembering her.

Something must have happened inside the home that day because an alert went out, and by early Thursday morning, EMS crews were heading to 5841 Rowsgate Lane.

What’s surprising is that the call had not come from a neighbor who heard something alarming. Neither Stephen nor Tracey had spoken to a dispatcher and asked for assistance.

Instead, one of the couple’s smartwatches detected a possible medical emergency and contacted 911 automatically. Reports differed slightly on whether the device registered a hard fall or an absence of pulse, but a sheriff’s office spokesperson said its alarm was not canceled.

When EMS workers arrived, they could not get inside the residence and requested help from law enforcement. New Hanover County sheriff’s deputies then entered through an open garage door.

Inside, they found Stephen and Tracey unresponsive, and both appeared to have suffered gunshot wounds, according to the local report. But the whole picture is much more heartbreaking.

Detectives Reached a Painful Conclusion

The New Hanover County Sheriff’s Office initially described the incident as isolated and assured locals there was no known danger to the surrounding community as detectives continued examining the evidence at the scene.

Finally, on Friday, July 31, the sheriff’s office announced what investigators believed had happened behind the closed doors.

“Based on the evidence gathered during the investigation, detectives believe this to be a murder-suicide in which the wife fatally shot her husband before taking her own life,” the agency said.

No possible motive was disclosed, and investigators did not publicly explain what evidence led them to that determination.

The sheriff’s office also stressed that its work had not ended with the announcement. The case remained active, and officials said more information would be provided if it became available.

The conclusion was especially difficult to reconcile with how others described the Willettses. In the memories shared after their deaths, they appeared devoted to each other and closely connected through a life built outdoors.

Their Life Together Often Led to the Water

Stephen and Tracey were well known within the Wrightsville Beach paddleboarding community. Jared Covington, the owner of Wrightsville SUP, said he had known them for more than a decade.

Their relationship with Jared grew from repeated days spent outdoors into something much closer. “We became like family, just sharing a love for being outside on the water, paddleboarding,” he said.

Jared remembered the couple as active and sociable, but his description also carried a more intimate observation: they appeared deeply committed to one another.

Friends saw something similar in Tracey’s treatment of the people around her. One told WWAY that Tracey consistently wanted to do what was right for her community.

Neighbor Andy Johnson said Stephen had retired from the Army. Because Andy had also served, the two men often stopped to talk in the afternoons. Andy remembered Stephen as physically fit and passionate about paddleboarding. “It’s just a sad situation,” he said.

Another neighbor, identified by WECT as Wyatt Patrick, said he crossed paths with Stephen nearly every day. Wyatt often encountered him while running or when they were both out walking their dogs.

Stephen would ask how Wyatt’s running was progressing or pause for a quick greeting. Wyatt described him as dependable in those everyday interactions and among the friendliest people he had met in the neighborhood.

WWAY identified another neighbor as Wyatt Heartline and reported a similar recollection. “One of the friendlier guys in our neighborhood, honestly, and he was always down to have a conversation,” he said. “I really liked him and it’s just kind of horrifying what just happened.”

Both neighbors who spoke with WECT said they had never met Tracey. Those who knew her personally, however, remembered a woman who checked on people and remained present in their lives.

That caring side appeared again in a relative’s tribute, but his memories also revealed that the Willettses had been carrying a sorrow far greater than many outside the family would have known.

Their Family Had Endured Two Earlier Losses

Fuller Royal identified Tracey as his second cousin and described Stephen as a lifelong friend. On July 30, he shared an old family photograph that he believed had been taken around 1999 or 2000, when all four members of the family were still together.

Fuller recalled Tracey visiting his parents to check on them. His memories of Stephen stretched back through their time at the University of North Carolina Wilmington and their work as EMTs with Whiteville Rescue.

The two men also shared the kind of ordinary memories that become precious after someone dies. They laughed together, watched “Caddyshack” repeatedly, and went to see “Taps” and “Excalibur” at least three times apiece.

Stephen even appeared in Fuller’s film, “James Bonded:000.” Yet the part of their history Fuller emphasized most strongly was Stephen’s work during emergencies.
